Go to your email, that you entered on the previous form, and check your inbox.

Silver Lake Camp will email you an electronic Docu Sign Waiver and Medical information form. Fill those out!

Heads up! Plan on 15 minutes to complete.You will need insurance, doctor, and prescription info. You need one waiver for each student.
